Required for All Claims

Every workers’ compensation claim must be accompanied by a drug and alcohol screening.

When to Report

Employees should report any work-related injury, illness, exposure, near-miss with injury, or incident that may require medical attention โ€” even if symptoms seem minor.

Under Missouri’s Division of Workers’ Compensation, notice to the employer should be in writing and include:

  • Date, time, and place of injury
  • Nature of the injury
  • Name and address of the injured person

Notify your on-duty supervisor or HR so authorized workers’ compensation medical care can be arranged.

๐Ÿ“ Declining Medical Treatment?

Complete both the Employee Incident Report and the Medical Treatment Declination Form. If symptoms worsen or you later want care, notify your supervisor immediately.

Important Notes on Declining Treatment

  • Declining treatment does not mean the incident should go unreported.
  • The declination form documents that authorized medical treatment was offered and declined.
  • Completing a declination form does not waive your right to report the incident.
  • If your symptoms worsen or you later decide you need medical care, notify the on-duty supervisor or HR immediately.

Information submitted will be shared only with those who need it to evaluate, report, administer, or respond to the incident โ€” including administration, HR, supervisors, the workers’ compensation carrier, medical providers, or regulators as required.

๐Ÿ›ก๏ธ Non-Retaliation

Employees are expected to report workplace injuries and incidents promptly. SFCAD prohibits retaliation for good-faith reporting of a workplace injury, illness, or safety concern.
